Hospice is a way of caring for the people who have a limited life expectancy due either to a specific disease process or overall medical decline. Emphasis is placed on the quality of life of the patient during end of life care. There is a nurse on call twenty-four hours per day seven days a week, even holidays. We at Amazing Grace Hospice Inc. are committed to ensuring you or your loved one gets the full compassionate & caring benefit of our services.

How do I know if I have hospice benefits?

Call us, we can do all of the footwork for you, or you contact your insurance yourself. If you have Medicare or Medical you have a hospice benefit.

Where is hospice provided?

Hospice is provided wherever a person calls home. This can be in their own home, home of a friend or relative, assisted living, skilled nursing facility & even non-conventional places.

Who decides it's time for Hospice?

The decision is ultimatley up to you or your decision maker. A physician may suggest it, but we can answer any questions you may have either by phone or in person.

Hospice Medical Director - Oversees & leads the team.

Registered Nurse - Assesses & manages pain & symptoms, coordinates care & services.

Licensed Vocational Nurses - Assists the physician & registered nurse. Assists in education & support of family.

Social Worker - Assesses family & patients emotional, social support, financial needs & provides counseling & bereavement support.

Volunteers - Our volunteers are trained to provide respite, socialization, support & relief for the caregivers.

Spirtual Counselor - Provides support & comfort directly or contacts local clergy at the request of the patient or family. Also provides bereavement.

Other Disciplines - Are available by consulation. Such as Dietitian, Physical Therapist, Occupational Therapist, Speech Therapist.

We care for you and those whom you love by providing the peace and support that allows you to experience life's final stage with comfort and dignity.
